This is a follow up to last week’s reminder about need for updated medical forms, including part C, for all Scouts and Scouters participating in summer camp and/or Yellowstone high adventure this summer. Here is a link for you, so you can copy the forms and fill them you, have your medical provider fill them out:http://www.scouting.org/filestore/HealthSafety/pdf/680-001_ABC.pdf
An attachment below is the PDF version of this form.
The following Scouts are eligible to have their yearly BSA required physical exam through the Troop this year (they had their exam last year with their primary care doctor). These exams will take place during Troop time, Tuesday May 31. Scouts participating are asked to pay $25, which will be a donation to Troop 282’s general fund (ISAs can be charged for this). Please let me know if you would like your Scout to participate:
The following Scouts had their last year’s exams completed in July (or more recently) and do not require an updated exam before summer camp. Updated physical exams (part C of the medical form completed in the past 12 months) are required for high adventure trips and extended trips to remote areas (72 hours or longer). Most of the Scouts listed below are also on the list above. Unless they are planning on going on a high-adventure next spring, there may not be a need for updated part c form before next summer.
If your Scout is not listed above, and you have not recently provided the Troop with a completed medical form that includes part C, please make sure to contact your primary care physician to schedule an exam and or complete the forms. If you have any questions about what forms are needed for you or your Scout, please email me.
Scouters going to summer camp and Yellowstone also require updated medical forms (if completed in July 2015 or sooner, you do not need update before summer camp), including part C. If you have any questions about whether your form needs to be updated, please feel free to email me.
Please feel free to email me if you have any questions or concerns about the above.
